Every Prime Rehab participant is allocated a Key Support Worker with an allied health background.
The Key Support Worker develops a detailed understanding of the participant's disability, health needs, goals, routines, equipment and allied health recommendations.

We don't believe in a one-size-fits-all approach to disability support.
Each participant's support team is developed according to their individual needs, preferences, goals, routines and circumstances. Where multiple support workers are required, the Key Support Worker helps ensure the team works consistently and understands how the participant prefers to be supported.

A recommendation from an allied health professional is only valuable if it can be consistently implemented in the participant's everyday life.
Prime Rehab's support teams work with the participant's existing allied health professionals to understand and implement relevant recommendations as part of daily support.

Our support workers are trained to be attentive to changes in a participant's physical presentation and wellbeing.
We can also support the implementation of established pressure care, repositioning and equipment recommendations provided by the participant's treating team.

For participants who have an established exercise or stretching program, Prime Rehab can support the consistent implementation of these programs within their everyday routine.
The focus is not simply on completing exercises, but on helping participants maintain physical capacity, mobility and participation over the longer term.

Prime Rehab believes effective disability support is most successful when everyone involved in a participant's care is working towards the same goals.
Our Key Support Workers maintain regular communication with the participant's Support Coordinator, family and broader allied health team, helping to ensure that the participant's support remains coordinated and responsive to their changing needs.
